Dear Anna ISD High School Parent or
Guardian:
Please allow me to introduce myself to you.
My name is Judy Smith. I have been a registered nurse since May, 1978. I
graduated from TWU School of Nursing with a Bachelor of Science Degree.
I worked at Parkland Memorial Hospital on a surgery and trauma floor for
one year. I then went to Baylor Richardson Medical Center and worked on
a post-op surgery/orthopedic floor until starting a family in 1985. I
have worked in the Anna School District for the past 13 years. I am
scheduled to be on the High School campus Monday through Friday,
7:55a.m.to 4p.m. If you have any questions or concerns about your

Anna ISD Medication Policy
If a student needs to take a prescription
medication at school it must meet the following criteria.
1. It must be in the original container
from the pharmacy with all of the necessary information on it,
including the student’s name, name of medication, dosage, and time to be
given. Any medication that is scheduled for once a day, twice a day or
three times a day, should be taken at home unless the doctor has
specified a certain time. An antibiotic that has been scheduled for
three times a day should be taken before school, after school, and at
bedtime. By spreading the doses out over a longer period they are more
effective.
2. The parent or guardian will need to
provide a written request for the student to take a medication at
school. (See attached)
Over the Counter Medication
The school district will not provide any
over the counter medication for students. If you feel your student may
occasionally need something for example, Tylenol, ibuprofen, or midol
etc., please send the medication in the original container and fill out
and sign a permission to dispense over the counter medication form. (See
attached)
Permission to Carry Inhaler
If your student uses an inhaler for asthma,
he/she needs to have the inhaler labeled with the pharmacy label on the
canister. The parent/guardian will then need to fill out the permission
form for the student to carry their inhaler. (See attached) Please
remind your student if they ever feel the need to take their rescue
inhaler more than once in a school day, to please come to the nurse’s
office and allow her to listen to their lungs and do an assessment.
Permission to Carry Epipen
If your student has life-threatening
allergic reactions to something, please notify the school nurse. If
they have been prescribed an Epipen, please fill out the medication
permission form and return to school. The Epipen must be properly
labeled with the student’s name and instructions for its use.
Immunization Requirements
In order to attend school each student must
have their immunization records up to date. They must be in compliance
with the requirements set down by the State of Texas. (See attached)
If your student is claiming a vaccine
exemption for reasons of conscience you must supply the school with an
original vaccine exemption affidavit form. To receive this form you
must submit your request to:

After the original vaccine exemption
affidavit form is signed an notarized, it must be submitted to the
student’s school. Photocopies of the vaccine exemption affidavit form
are not valid. If your child changes school, the vaccine exemption
affidavit is part of the child’s school records and should be sent to
the new school with other school records. Each child’s individual
vaccine exemption affidavit is good for two years from the date
notarized.
If a child currently has a religious
exemption for vaccinations on file with the school, filed prior to
September 1, 2003, they do not need a new vaccine exemption affidavit
form. The religious exemption on file remains valid.
